ABSTRACT

Dental fluorosis can cause changes in the enamel surface, conditioning its functionality and esthetics. The application of dental adhesives is a treatment option; however, their use on fluorotic teeth can result in limitations. The aim of this study was to compare the shear bond strength of two different adhesives, one with 10-MDP and one without 10-MDP, in different degrees of dental fluorosis.This is an in vitro study on dental enamel samples, a total of 180 samples with the inclusion criteria were treated, randomly divided into two groups of 90, according to the type of dental adhesive, where each group was again divided into three groups of 30 samples, representing different degrees of dental fluorosis according to the Thylstrup-Fejerskov index (Group I: TF1 and TF2; Group II: TF3; Group III: TF4). Normality tests, two-factor ANOVA, and post-hoc tests were used to determine differences between the groups, with a significance level of 95%. As results, a statistically significant difference was shown between the use of dental adhesive with 10-MDP and the three groups of dental fluorosis (p=0.011), in addition, a Tukey post-hoc test on the groups treated with 10-MDP adhesive revealed a statistically significant difference between group I versus group II, and group I versus group III, (p=<0.05). It is concluded that the use of adhesive systems with 10-MDP presents a better shear bond strength on enamel with dental fluorosis grades I and II in the Thylstrup-Fejerskov index.


La fluorosis dental puede ocasionar cambios en la superficie del esmalte, condicionando su funcionalidad y estética, la aplicación de adhesivos dentales es una opción de tratamiento, sin embargo, su uso en dientes fluoroticos puede resultar en limitaciones. El objetivo de este estudio consistió en comparar la fuerza de adhesión a la cizalladura de dos diferentes adhesivos, uno con 10-MDP y otro sin 10-MDP, en diferentes grados de fluorosis dental. Se trata de un estudio in vitro en muestras de esmalte dental, un total de 180 muestras con los criterios de inclusión fueron tratadas, aleatoriamente divididas en dos grupos de 90, de acuerdo al tipo de adhesivo dental, donde cada grupo fue dividido nuevamente en tres grupos de 30 muestras, representando diferentes grados de fluorosis dental según el índice de Thylstrup- Fejerskov (Grupo I: TF1 y TF2; Grupo II: TF3; Grupo III: TF4). Para determinar diferencias entre los grupos se utilizaron pruebas de normalidad, ANOVA de dos factores, y pruebas post-hoc, con un nivel de significancia de 95%. Como resultados, se mostró una diferencia estadísticamente significativa entre el uso de adhesivo dental con 10- MDP y los tres grupos de fluorosis dental (p=0.011), además, una prueba post hoc de Tukey sobre los grupos tratados con adhesivo 10-MDP revelaron una diferencia estadísticamente significativa entre el grupo I frente al grupo II, y el grupo I frente al grupo III, (p=<0.05) Se concluye que el uso de sistemas adhesivos con 10-MDP presentan una mejor resistencia de adhesión al cizallamiento en esmalte con grados de fluorosis dental I y II en el índice de Thylstrup-Fejerskov

ABSTRACT

La adhesión a dentina con sistemas adhesivos polimé-ricos representa un desafío que surge de la necesi-dad de vincular un sustrato dentario heterogéneo y variable con materiales que presentan todavía cier-tos aspectos a atender para poder conseguir el máxi-mo desempeño. El propósito del presente artículo es evaluar algunas de las estrategias propuestas para mejorar la adhesión a dentina, y sugerir un protocolo de trabajo con los diferentes tipos de sistemas adhesivos (AU)


Dentin bonding with polymeric adhesive systems represents a challenge that arises from the need to link a heterogeneous and variable dental substrate with materials that still have certain aspects to be addressed in order to achieve maximum performance. The aim of this article is to evaluate some of the proposed strategies to improve dentin bonding and to suggest a protocol for each different type of bonding systems (AU)

ABSTRACT

O objetivo deste estudo clínico, controlado, randomizado, cego e prospectivo foi de avaliar a efetividade clínica de restaurações em lesões não cariosas (cavidades Classe V) realizadas em superfícies dentais pré-tratadas ou não com laser de Er:YAG e utilizando dois sistemas adesivos autocondicionantes. Este estudo foi realizado com um total de 91 lesões e divididas em 4 grupos (n = 23) :G1(FL): Sem pré-tratamento e utilizando o adesiso FL-Bond II/Shofu; G2 (CF): sem pré-tratamento e utilizando o adesivo Clearfil SE Bond 2/Kuraray Noritake; G3 (Laser + FL): pré-tratamento associado ao uso do FL-Bond II/Shofu e G4 (Laser + CF): pré-tratamento associado ao uso do Clearfil SE Bond 2/Kuraray Noritake Os dentes avaliados foram os pré-molares de ambas as arcadas. A avaliação foi feita imediatamente após o tratamento e ao longo do tempo, em 3, 6 e 12 meses após a finalização da restauração. Para a avaliação da sensibilidade foi considerado a EVA (Escala Visual Analógica) e para a qualidade da restauração o método do USPHS modificado, considerando a avaliação de perda de retenção, integridade marginal, pigmentação marginal, presença de lesão de cárie em margem de restauração, vitalidade pulpar e sensibilidade pós-operatória. Os dados foram registrados em tabelas de frequência, e os escores transformados em postos para a aplicação do teste ANOVA. A significância estatística foi considerada para valores de p<0,05. Para os resultados da avaliação de sensibilidade houve uma diferença estatística significante da sensibilidade ao longo do tempo (p<0,001), sendo que essa diferença se deu a partir da avaliação T1 (48 horas) em todos os grupos. Em relação aos critérios avaliados no USPHS, foram encontrados variações estatísticas em alguns tópicos: Pigmentação marginal - G1 (FL) (p=0,041) e G2 ( Laser + FL) (p=0,009) e Integridade marginal G1(FL)( p=0,036). Os demais grupos e critérios não apresentaram variações estatísticas significantes.Pré-tratamentos com laser Er:YAG podem ser uma alternativa viável em restaurações adesivas do tipo classe V; no entanto, nenhum efeito sinérgico de ambos os tratamentos combinados foi observado.


The objective of this clinical, controlled, randomized, blinded and prospective study was to evaluate the clinical effectiveness of restorations in non-carious lesions (Class V cavities) performed on dental surfaces pre-treated or not with Er:YAG laser and using two adhesive systems self-conditioning. This study was carried out with a total of 91 lesions and divided into 4 groups (n = 23) :G1(FL): No pre-treatment and using the FL-Bond II/Shofu adhesive; G2 (CF): without pre-treatment and using Clearfil SE Bond 2/Kuraray Noritake adhesive; G3 (Laser + FL): pre-treatment associated with the use of FL-Bond II/Shofu and G4 (Laser + CF): pre-treatment associated with the use of Clearfil SE Bond 2/Kuraray Noritake The teeth evaluated were the premolars of both arcades. Assessment was performed immediately after treatment and over time, at 3, 6, and 12 months after completion of the restoration. The VAS (Visual Analogue Scale) was used for the sensitivity assessment and the modified USPHS method for the quality of the restoration, considering the assessment of loss of retention, marginal integrity, marginal pigmentation, presence of caries in the restoration margin. , pulp vitality and postoperative sensitivity. Data were recorded in frequency tables, and the scores were transformed into ranks for the application of the ANOVA test. Statistical significance was considered for values of p<0.05. For the results of the sensitivity evaluation, there was a statistically significant difference in sensitivity over time (p<0.001), and this difference occurred from the T1 evaluation (48 hours) in all groups. Regarding the criteria evaluated at the USPHS, statistical variations were found in some topics: Marginal pigmentation - G1 (FL) (p=0.041) and G2 (Laser + FL) (p=0.009) and Marginal integrity - G1(FL)( p =0.036). The other groups and criteria did not present significant statistical variations. Pretreatments with Er:YAG laser can be a viable alternative in class V adhesive restorations; however, no synergistic effect of both treatments combined was observed.

RESUMEN La preservación de tejido dental sano contribuye a la práctica de una Odontología conservadora, permitiendo la aplicación de los principios biológicos y mecánicos de una forma más predecible. Las preparaciones cavitarias mínimamente invasivas son el complemento de la correcta selección de materiales adhesivos y de resina compuesta. La correcta aplicación de un protocolo adhesivo nos proporcionará resultados estéticos imperceptibles y un longevo desempeño clínico.


ABSTRACT The preservation of healthy dental structures contributes to the practice of a conservative dentistry. This concept involves the implementation of biological and mechanical principles in a more predictable manner. Minimally invasive cavity preparations are the complement to the selection of adhesive and composite resin materials. The correct application of an adhesive protocol will provide imperceptible aesthetic restorations and adequate clinical performance.

Objective: to compare, through a systematic review and a meta-analysis, the clinical effect of the adhesive strategies of universal adhesives (UA) in the treatment of non-carious cervical lesions (NCCLs). material and method: a search of the literature was carried out up to january 2018, in the biomedical databases: Pubmed, Embase, Scielo, Science Direct, SIGLE, LILACS, BBO, Google Scholar and the Central Register of Cochrane Clinical Trials. the selection criteria of the studies were as: randomized clinical trials, with a maximum age of 5 years and which report the clinical effects (marginal adaptation, discoloration or marginal staining, presence of secondary caries, postoperative sensitivity, retention and fractures) of the UA in the treatment of NCCLs. the risk of study bias was analyzed through the Cochrane Handbook of systematic reviews of interventions. results: the search strategy resulted in eight articles that reported no difference in marginal adaptation, discoloration or marginal staining, presence of secondary caries and postoperative sensitivity among the adhesive strategies of the UA; however they reported a difference between the retention and the presence of fractures, with the conventional adhesive strategy resulting in a better clinical effect. conclusion: the reviewed literature suggests that the conventional adhesive strategy of UAs results in greater retention and absence of fractures in the treatment of NCCLs.

ABSTRACT

Abstract This randomized clinical trial aimed to assess the efficacy of sealing occlusal carious lesions in permanent teeth. The sample consisted of 54 occlusal carious lesions in permanent molars and premolars of 49 patients aged 8–43 years (median: 19 years). The inclusion criteria comprised the presence of a cavity with no access allowing biofilm control. The maximum depth of the lesion was the middle third of the dentin thickness, as assessed by bitewing radiography. The teeth were randomly assigned to sealant treatment (n = 28) or restorative treatment (n = 26). Clinical and radiographic examinations were performed after 1 year and after 3–4 years. The outcomes depended on the clinical performance of the sealant/restoration and the control of caries progression observed radiographically. Survival analysis was performed to assess success rates. Over the 3-4 years of monitoring, 2 sealants were totally lost, 1 needed repair, and 1 showed caries progression, totaling 4 failures in the sealant group. In the restoration group, 1 failure was observed (in need of repair). The success rates were 76% and 94% in the sealant and the restoration groups, respectively (p > 0.05). The sealing of occlusal carious lesions in permanent teeth succeeded in controlling caries over a 3–4-year period. However, sealed carious lesions require patient compliance in attending regular follow-ups to control the occurrence of clinical failures of the sealants.

Objetivo: avaliar a influencia da contaminação cavitária na adaptação marginal das restauraçõesde resina composta e a associação da lisozima ao sistema adesivo. Materiais e Métodos:Foram selecionados 40 terceiros molares permanentes e confeccionados preparos cavitários classeV com pontas diamantadas cilíndricas 1090 (4 mm de largura, 6 mm de comprimento e 1,5 mmde profundidade). Todos os dentes foram esterilizados e divididos aleatoriamente em 4 grupos(n=10): G1: sistema adesivo (SA) e restauração com resina composta (RC); G2: cavidadescontaminadas com 10 μl de Streptococcus mutans (0,5 de Mc Farland), SA e RC; G3:sistema adesivo associado com lisozima (ADL) e RC e G4: cavidades contaminadas com 10 μlde Streptococcus Mutans (0,5 de Mc Farland), ADL e RC. A lisozima foi adicionada ao SA com ummicrobrush embebido no SA e na lisozima em pó. Os dentes foram impermeabilizados e imersosem azul de metileno por 4 horas a 37ºC. Os dentes foram seccionados e fotografados. Resultados:A avaliação da microinfiltração foi feita por dois avaliadores utilizando o Programa Tpsdig. Paraavaliar a calibração entre os examinadores, foi utilizado o teste de Correlação de Pearson. As médiasdas porcentagens de fendas e bolhas foram submetidas ao teste de Kruskal Wallis com 5% designificância. Conclusão: a contaminação cavitária com Streptococcus mutans acarretou na formaçãode fendas entre a resina composta e as margens cavitárias. A lisozima associada ao sistemaadesivo não influenciou na formação de fendas e bolhas nas restaurações de resina composta.


Objective: evaluate the influence of cavity contamination on marginal adaptation of compositerestorations and the association of lysozyme to the adhesive system. Materials and Methods:Were selected 40 permanent third molars and made cavity preparations class V with cylindricaldiamond burs 1090 (4 mm wide, 6 mm long and 2 mm deep). All teeth were sterilized and randomlydivided into 4 groups (n = 10): G1: adhesive system (SA) and restoration with compositeresin (CR); G2: cavity contaminated with 10 μl of Streptococcus mutans (0.5 McFarland), SA andRC; G3: adhesive system associated with lysozyme (ADL) and RC and G4: cavity contaminated with10 μl of Streptococcus mutans (0.5 Mc Farland), ADL and RC. Lysozyme was added to the SA witha microbrush embedded in the SA and powdered lysozyme. The teeth were sealed and immersedin a methylene blue for 4 hours at 37 ° C. The teeth were sectioned and photographed. Results:The evaluation of microleakage was made by two reviewers using the Tpsdig Program. To assessthe calibration of examiners, we used the Pearson correlation test. The mean percentages of cracksand bubbles were submitted to Kruskal Wallis test at 5% significance level. Conclusion: the cavityStreptococcus mutans contamination resulted in the formation of cracks between the compositeresin and the cavity margins. Lysozyme associated with the adhesive system did not influence theformation of cracks and bubbles in the composite restorations.

Aim: The aim of this in vitro study was to observe and compare the microstructure of the adhesive interface between resin and dentin treated with Papacarie® using scanning electron microscopy (SEM). Materials and Methods: Totally, 10 human dentin slabs were randomly distributed into two groups. The control group (n = 5) was subjected to etching with 37% phosphoric acid for 20 s and washed for 30 s, dried with absorbent paper, and the bonding agent was applied along with low viscosity resin. In turn, the experimental group (n = 5) was subjected to the same procedure, but Papacarie® (Fórmula and Ação, São Paulo, Brazil) was added for 30 s prior to etching. Adper™ Single Bond 2 (3M ESPE, São Paulo, Brazil) adhesive was applied to both groups following manufacturer instructions. The specimens were prepared for observation under SEM with ×1.000, ×2.000, ×2.200 and ×5.000 magnification. The micrographs were evaluated with respect to the formation of the hybrid layer, thickness, shape and length of the tags and microtags. Results: In the experimental group there was the formation of more fine hybrid layer and tags with average of similar length to the control group; microtags in less number and without formation of lateral branches. The resin tags presented conical, smooth and uniform characteristics. Conclusions: When Papacarie® was used prior to the application of a bonding agent it could interfere with the formation of the hybrid layer without changing the length of the tag. Moreover, the morphology in the experimental group was found to be more uniform and regular.

Objetivo: Avaliar a nanodureza (Nn) e o módulo de elasticidade (ME) da região de adesão quando utilizados diferentes sistemas adesivos e correlacionar com a resistência adesiva (RA) à dentina. Método: Foram medidos a nanodureza e o módulo de elasticidade da resina composta junto à área de união, da camada de adesivo (Ad), da camada híbrida (Ch), e da dentina adjacente utilizando testes de nanoendentação. Quarenta molares (n=10) foram preparados de modo a obterem-se superfícies planas em dentina, sobre as quais os sistemas adesivos foram aplicados (Adper Scotchbond Multiuso, Adper Single Bond 2, Adper SE Plus e Clearfil SE Bond). Sobre essas superfícies foi inserida a resina composta. Os dentes restaurados foram seccionados para obterem-se corpos-de-prova com área aderida de 1mm2. Um palito de cada dente foi selecionado para análise da nanodureza e do módulo de elasticidade da interface adesiva. Os demais tiveram a resistência de união testada através do teste de microtração. Resultados: O teste ANOVA detectou diferença estatística entre todos os grupos, exceto para a variável nanodureza da camada híbrida. O teste de correlação de Pearson detectou significância para o módulo de elasticidade. Conclusões: Pôde-se concluir que as propriedades mecânicas (resistência adesiva, módulo de elasticidade e nanodureza do sistema adesivo e da camada híbrida) variam dependendo do sistema adesivo. Quanto maior o módulo de elasticidade da camada híbrida de um sistema adesivo maior é sua resistência adesiva e menor o módulo de elasticidade da camada de adesivo. Pôde observar que não houve correlação entre nanodureza e a resistência adesiva.


To evaluate the nanohardness (Nn) and the modulus of elasticity (ME) in the region of adhesion when used different adhesive systems and correlate with the bond strength to dentin (RA). We measured the nanohardness and the modulus of elasticity of the resin composed with the area of bond of the adhesive layer (Ad), hybrid layer (Ch) and adjacent dentin using nanodentition tests. Forty molars (n = 10) were prepared in order to obtain flat dentin surfaces on which the adhesive systems were applied (Adper Scotchbond Multipurpose, Single Bond 2, Adper SE Plus and Clearfil SE Bond). On these surfaces was inserted a composite resin. The restored teeth were sectioned to obtain body-of-proof with adhered area of 1mm2. A stick of each teeth was selected for the nanohardness analysis and the modulus of elasticity of the adhesive interface. The others had the bond strength tested using the microtensile test. The ANOVA detected statistical difference between all groups, except for the variable nanohardness of the hybrid layer. The Pearson’s correlation test found significance for the modulus of elasticity. It was concluded that the mechanical properties (bond strength, the modulus of elasticity and nanohardness of the adhesive system and the hybrid layer) vary depending on the adhesive system. The higher the modulus of elasticity of the hybrid layer of an adhesive system a greater adhesive strength will be and lower the modulus of elasticity of the adhesive layer. It can be observed that there was no correlation between nanohardness and bond strength.

ABSTRACT

A resistência de união (RU) dos materiais deve ser avaliada após a degradação da interface adesiva, tentando simular um processo que ocorre naturalmente ao longo do tempo na cavidade bucal. O presente estudo avaliou a degradação da interface adesiva em diferentes materiais utilizando o modelo de envelhecimento in vitro e in situ em dentina hígida e cariada. Noventa segundos molares decíduos hígidos e sem defeitos de esmalte, foram aleatoriamente divididos em dois grupos de acordo com o substrato a ser testado (hígido e com lesão de cárie induzida). A seguir, cada um dos grupos foi subdividido aleatoriamente de acordo com o material restaurador (n=15): resina composta associada ao sistema adesivo - RC, cimento de ionômero de vidro modificado por resina - CIVMR e cimento de ionômero de vidro de alta viscosidade - CIVAV. A superfície da dentina foi preparada e com o auxílio de cânulas de polietileno, foram confeccionados os espécimes com os materiais restauradores. Partes de um mesmo dente foram submetidas a cada envelhecimento: in vitro (24 horas e 6 meses) - armazenados em água destilada à 37°C; e in situ (uso de dispositivo intraoral palatino por voluntários em uma fase experimental de 7 dias). Os espécimes foram submetidos ao teste de microcisalhamento (MPa) para avaliação da RU, seguido de análise do padrão de fratura em estereomicroscópio (400X). Os dados foram submetidos a análise de variância de medidas repetidas seguida pelo teste de Bonferroni (? = 5%). De forma geral, o armazenamento por seis meses em água degradou o grupo CIVMR em dentina hígida (<0,001) e RC em dentina cariada (p=0,007), para os outros materiais não houve diferença estatística (p=1,00)...


Materials bond strength (BS) should be evaluated after degradation of the adhesive interface, simulating the intra-oral enviroment. This study assessed the degradation of the bonding interface of differents materials using in vitro and in situ models of aging in sound and caries - affected primary dentin. Ninety sound second primary molars without enamel defects were randomly assigned into two groups (sound and induced carious) according to the substrate to be tested. Then each group was subdivided according to the restorative material (n=15): composite resin associated with the adhesive system - CR, resin-modified glass ionomer cement - RMGIC or high-viscous glass ionomer cement- HVGIC. Dentin surface was prepared and with the aid of polyethylene tubes specimens of each restorative materials were prepared. Parts of a same tooth were subjected to in vitro and in situ models of aging. For in vitro model, the specimens were stored in distilled water at 37°C during 24 hours per 6 months; and for in situ model an intraoral palatal device was confectioned and used by volunteers in an experimental phase for 7 days.Specimens were submitted to microshear test (MPa) for BS evaluation and then stereomicroscopy (400X) analys of failure mode. A hidroxiapatita (HA) é o principal constituinte mineral de ossos e dentes, e têm sido foco de grande interesse na biomedicina devido sua excelente biocompatibilidade e bioatividade, em especial para aplicação em implantes dentários e ortopedia. Na odontologia estudos mostram o sucesso do uso de nanopartículas (NP) de HA para tratamento de hipersensibilidade dentinária, remineralização de lesões de cárie em esmalte, reparação e prevenção de lesões de erosão iniciais. No entanto, existem evidências que o tamanho do cristal desempenha um papel essencial na formação dos tecidos duros e possui significante influência nas propriedades intrínsecas destes cristais, incluindo solubilidade e, consequentemente, biocompatibilidade. Este trabalho teve como objetivo verificar a síntese e caracterização de nanopartículas de hidroxiapatita funcionalizadas e avaliar a influência de sua aplicação no processo adesivo em dentina. Nanopartículas de HA com diferentes tamanhos e morfologia foram sintetizadas pelo método de precipitação em solução aquosa. Foi investigada a influência de dois novos ligantes, ácido deoxicólico (AD) e ácido abiético (AB), no preparo da NP de HA. As NP foram caracterizadas através das análises de espectroscopia por dispersão de raios X (EDX), Difratometria de Raios X (DRX), Espalhamento de Luz Dinâmica (DLS), Microscopia Eletrônica de Transmissão (MET), Microscopia Eletrônica de Varredura (MEV), Microscopia de Força Atômica (AFM). Hydroxyapatite (HA) is the major mineral component of bones and teeth, and has been the focus of great interest in biomedicine because of its excellent biocompatibility and bioactivity, in particular for application in dental and orthopedic implants. In dentistry studies have shown the successful use of nanoparticles (NP) of HA for the treatment of dentin hypersensitivity, remineralization of caries lesions in enamel, repair and injury prevention of initial erosion. However, there is evidence that the size of the crystal plays an essential role in the formation of hard tissues and has significant influence on the intrinsic properties of these crystals, including solubility and biocompatibility. This study aims to verify the possibility of functionalized hydroxyapatite nanoparticles and evaluate possible benefits of using these during the adhesive process in dentin. Este estudo avaliou o efeito do desafio erosivo nas propriedades e na estabilidade de união de materiais restauradores ao esmalte e dentina decídua. Duzentos e quarenta molares decíduos foram alocados em 24 grupos, de acordo com substrato - esmalte ou dentina, condição prévia do substrato - hígido ou erodido, material restaurador - resina composta associada ao sistema adesivo - RC, cimento de ionômero de vidro de alta viscosidade - CIVAV, ou modificado por resina - CIVMR, e tratamento após procedimento adesivo - controle (imersão em solução salina/7dias) ou desafio erosivo (imersão em bebida a base de cola por 5 min/3x ao dia/7 dias). Os dentes dos grupos esmalte e dentina erodidos foram submetidos ao desafio erosivo previamente aos procedimentos restauradores. Cânulas de polietileno foram posicionadas sobre superfícies planas dos substratos pré-tratados, para então serem preenchidas por um dos materiais avaliados. Após armazenamento por 24h a 37°C, os espécimes foram submetidos a um dos tratamentos propostos. Metade dos espécimes foi submetida imediatamente ao teste de microcisalhamento e a outra metade após 12 meses de armazenamento em água a 37ºC. Posteriormente, a análise do padrão de fratura foi realizada em estereomicroscópio (400X). Para a análise de resistência à flexão (RF) e rugosidade superficial (RS), trinta corpos de prova foram confeccionados (10x2x1mm). Após armazenamento por 24h, a leitura inicial de RS de dez corpos de prova selecionados aleatoriamente foi obtida. Os corpos de prova foram então imersos em um dos tratamentos propostos - desafio erosivo ou solução salina, para então ser realizada a segunda leitura. Decorrido esta análise, os corpos de prova foram submetidos ao teste de RF (1mm/min). Os valores de resistência de união (RU), bem como de RS e RF obtidos foram submetidos a Análise de Variância e Teste de Tukey (?=5%). This study evaluated the effect of erosive challenge on the properties and bond stability of restorative materials to primary enamel and dentin. Two hundred and forty primary molars were assigned into 24 groups, in according to the substrate - enamel or dentin, the precondition of substrate - sound or eroded, the restorative material - resin composite associated to adhesive system - RC, high-viscous glass ionomer cement- HVGIC or resin-modified glass ionomer cement - RMGIC, e the treatment after restorative procedure - control (immersion in saline solution/7 days) or erosive challenge (immersion in cola-based drink for 5 min/3x per day/7 days). Teeth from the eroded dentin and enamel groups were subjected to erosive challenge prior to bonding procedures. Polyethylene tubes were placed on flat surfaces of the pretreated substrates, and then were filled up with one of evaluated materials. After storage for 24h at 37ºC, the specimens were submitted to one of the proposed treatments. Half of the specimens were immediately submitted to the microshear bond strength test, and the other half after 12 months of water storage at 37ºC. Subsequently, the failures pattern evaluation was performed in stereomicroscopy (400X). For the assessment of flexural strength (FS) and superficial roughness (SR), thirty specimens were built up (10x2x1mm). ABSTRACT

The aim of this study was to evaluate the effect of treating intraradicular dentin with irrigating solutions on the retention of glass-fiber posts luted with self-adhesive resin cement. Bovine incisors were endodontically treated, and 9-mm-deep postholes were prepared. Before inserting the cement, the root canals were irrigated with various solutions: 11.5% polyacrylic acid for 30 s, 17% EDTA for 60 s, or 5% NaOCl for 60 s, respectively. Irrigation with distilled water was used in the control group. After all specimens had been rinsed with distilled water, the excess moisture was removed and the posts were luted using either BisCem (Bisco) or RelyX Unicem clicker (3M ESPE). Seven days after luting, the specimens were sectioned transversally into 1-mm-thick slices, which were submitted to push-out testing on a mechanical testing machine. Bond strength data (n = 6 per group) were analyzed by two-way ANOVA and Student-Newman-Keuls' test (α = 0.05). For Unicem, EDTA showed lower bond strength than the other solutions, which had similar results. For BisCem, EDTA showed higher bond strength than the other treatments, while application of NaOCl yielded higher bond strength than polyacrylic acid whereas the control group had intermediate results. In conclusion, irrigating root canals before insertion of self-adhesive resin cements, especially EDTA, might interfere with retention of the fiber posts.

ABSTRACT

Este estudo avaliou in vitro a dureza da camada de adesivo e da camada híbrida de uma interface entre resina composta e dentina humana por meio de ensaios de nanodureza (DUH). Foi avaliada a resistência de união por meio de testes de microcisalhamento (MC) com o objetivo de observar a relação desta resistência com a propriedade da dureza. Os fatores de variação compreendem quatro sistemas adesivos (Adper Scotchbond Multiuso, Adper Single Bond 2, Clearfil SE Bond e Single Bond Universal) e duas profundidades dentinárias (dentina rasa e dentina profunda). As unidades experimentais foram compostas por 144 fragmentos dentais obtidos da face vestibular de molares humanos, os quais foram distribuídos em 16 grupos, 8 grupos com n=6 para os ensaios de DUH e 8 grupos com n=12 para os ensaios de MC de forma que os quatro sistemas adesivos foram aplicados sobre fragmentos de dentina rasa e de dentina profunda. Após a confecção das restaurações em resina composta e 24h de armazenamento em estufa a 37°C, os ensaios de DUH foram realizados no ultramicrodurômetro DUH-211S (Shimadzu) com carga de 1mN/mseg. Os testes de MC foram realizados na máquina de ensaios universais Instron 5942(Instron Co) com velocidade de 1mm/min seguido da análise do padrão de fratura em microscópio digital. A análise estatística foi realizada por ANOVA (p<0,01%), Tukey e correlação de Pearson (p<0,05%). O sistema SB Universal (M=31,82±4,72) evidenciou os maiores valores de resistência de união e o Clearfil SE (M=26,75±3,73) os menores.


O Single Bond 2 (M=28,83±4,07) e Scotchbond MP (M=28,53±4,61) não apresentaram diferenças entre si e obtiveram valores intermediários, não se diferenciando do maior e nem do menor valor. A resistência de união na dentina rasa (M=30,45±4,40) foi maior que em dentina profunda (M=27,52±4,01). Os sistemas SB Universal (M=22,57±2,42) e Scotchbond MP (M=22,13±2,89) não apresentaram diferença entre si e evidenciaram valores de dureza na camada de adesivo maiores que o Clearfil SE (M= 19,11±1,23). O sistema Single Bond 2 (M=20,11±1,49) apresentou valores intermediários de dureza e não foi diferente do maior e do menor valor. A camada híbrida em dentina rasa (M=38,23±3,08) apresentou maior dureza que a camada híbrida em dentina profunda (M=33,09±1,85). Pode-se concluir que tanto o sistema adesivo utilizado quanto a profundidade dentinária influenciaram nos resultados de resistência adesiva. A dureza da camada de adesivo varia conforme o sistema utilizado sendo que os adesivos SB Universal e o Scotchbond MP apresentaram os maiores valores de dureza. A dureza da camada híbrida foi influenciada pela profundidade dentinária do substrato. Foi observada uma correlação direta entre a dureza da camada de adesivo e a resistência adesiva.


This study evaluated in vitro the hardness of the adhesive layer and the hybrid layer interface between composite and human dentin using nanohardness test (DUH). Was evaluated the bond strength by microshear test (MC) in order to observe the relationship of this resistance with the hardness property. The variation factors consist of four adhesive systems (Adper Scotchbond Multi-Purpose, Adper Single Bond 2, Clearfil SE Bond and Single Bond Universal) and two dentin depths (superficial and deep dentin). The experimental units consisted of 144 dental fragments obtained from molars vestibular area, which were divided into 16 groups, 8 groups with n = 6 for DUH test and 8 groups with n = 12 for the MC test, so the four adhesive systems were applied on dentin fragments of superficial and deep dentin. After the construction of the composite resin restorations and 24h storage at 37 °C, DUH analysis were performed using a nanoindentation device DUH-211S (Shimadzu) with load of 1mN/mseg. The MC tests were performed in a universal testing machine Instron 5942 (Instron Co) with speed 1mm/min followed by analysis of the failure site in digital microscope. Statistical analysis was performed by ANOVA (p <0.01%), Tukey, and Pearson correlation (p <0.05%). The system SB Universal (M = 31.82 ± 4.72) showed higher values of bond strength and Clearfil SE (M = 26.75 ± 3.73) lower.


The Single Bond 2 (M = 28.83 ± 4.07) and Scotchbond MP (M = 28.53 ± 4.61) did not differ among themselves and had intermediate values, not differentiating the largest and the smallest value. The bond strength of superficial dentin (M = 30.45 ± 4.40) was higher than in deep dentin (M = 27.52 ± 4.01.) Universal Systems SB (M = 22.57 ± 2.42) and Scotchbond MP (M = 22.13 ± 2.89) did not differ among themselves and showed hardness values in the adhesive layer higher than Clearfil SE (M = 19.11 ± 1.23.) The system Single Bond 2 (M = 20.11 ± 1.49) showed intermediate values of hardness and was not different from the largest and smallest value. The hybrid layer in superficial dentin (M = 38.23 ± 3.08) was harder than the hybrid layer in depth dentin (M = 33.09 ± 1.85.) It can be concluded that both the adhesive system used as the dentin depth influenced the bond strength. The hardness of the adhesive layer varies depending on the system used and the SB Universal adhesive and Scotchbond MP showed higher hardness values. The hardness of the hybrid layer was influenced by the depth of the dentin substrate. We observed a direct correlation between the hardness of the adhesive layer and the bond strength.

In vitro studies have recommended enamel-etching before luting indirect restorations with self-adhesive resin cement. However, there is no scientific proof that this procedure has any clinical relevance. Thus, the aim of this study was to evaluate the effect of enamel-etching on the marginal quality of indirect composite resin restorations (inlay/onlay) using the self-adhesive cement RelyX Unicem clicker. Forty-two posterior teeth were selected from 25 patients that presented one or two teeth with large restorations in need of replacement. All teeth were prepared by the same operator and impressed with polyvinylsiloxane material. The composite resin restorations were built over plaster casts using the incremental technique with a LED device for light-curing the increments. The inlays/onlays were cemented according to one of the following protocols: 1. Etched group (ETR) - selective enamel phosphoric-acid etching + RelyX Unicem clicker; 2. Non-etched group (NER) - RelyX Unicem clicker alone. Each surface was light-cured for 60 s. Recalls occurred after 1 week (baseline), 6 and 12 months when the teeth were evaluated according to previously established scores (modified USPHS criteria). The clinical analysis showed little or no visible changes in the marginal quality after 1 year, even though a probe could detect all the restorations margins. Statistical analysis (Fisher’s exact test, p<0.05) detected no differences between the groups after 12 months. No restorations failed and no secondary caries were found. Enamel acid etching had no clinical relevance on the marginal quality of indirect composite resin restorations luted with RelyX Unicem clicker after 1 year of follow up.


Estudos in vitro têm recomendado condicionamento ácido do esmalte antes da cimentação de restaurações indiretas com cimento resinoso autoadesivo. Entretanto, não há nenhuma prova científica de que este procedimento tenha relevância clínica. Assim, o objetivo deste estudo foi avaliar o efeito do condicionamento ácido do esmalte na qualidade marginal de restaurações indiretas de resina composta (inlay/onlay) usando o cimento autoadesivo RelyX Unicem clicker. Quarenta e dois dentes posteriores foram selecionados de 25 pacientes com 1 ou 2 restaurações amplas com necessidade de substituição. Todos os dentes foram preparados pelo mesmo operador e impressos com material à base de polivilsiloxano. As restaurações de resina composta foram construídas sobre troquel de gesso usando a técnica de colocação em camadas e polimerizadas usando aparelho de luz emitida por diodo (LED). Osinlays/onlays foram cimentados de acordo com um dos protocolos: 1. Grupo condicionado (ETR)- condicionamento seletivo do esmalte com ácido fosfórico + cimentação com RelyX Unicem clicker; 2. Grupo não condicionado (NER)- cimentação com RelyX Unicem clicker somente. Cada superfície foi fotoativada por 60 s. Os retornos para acompanhamento foram feitos após 1 semana (baseline), 6 e 12 meses onde os dentes foram avaliados de acordo com critérios previamente estabelecidos (USPHS Modificado). A análise clínica mostrou pouca ou nenhuma alteração visível na qualidade marginal após 1 ano, porém uma sonda clínica pode detectar todas as margens restauradoras. A análise estatística (Teste exato de Fisher, p<0,05) não detectou diferença entre os grupos após 12 meses. Nenhuma restauração falhou e nenhuma cárie secundária foi encontrada. O condicionamento ácido do esmalte não teve relevância clínica na qualidade marginal de restaurações indiretas de resina composta cimentadas com RelyX Unicem clicker após 1 ano de acompanhamento.

OBJECTIVES: Adhesive systems are continuously being introduced to Dentistry, unfortunately often without sufficient clinical validation. The aim of this study was to evaluate the clinical performance of cervical restorations done with three different adhesive systems. MATERIAL AND METHODS: 158 non-carious cervical lesions of 23 patients were restored with a nanofilled composite resin (Filtek Supreme, 3M/ESPE) combined with Single Bond (3M/ESPE, group SI), Clearfil SE (Kuraray Medical Inc., group CL) and Xeno III (De Trey Dentsply, group XE). In groups SI-B, CL-B and XE-B, the outer surface of the sclerotic dentin was removed by roughening with a diamond bur before application of the respective adhesive systems. In groups CL-BP and XE-BP, after removal of the outer surface of the sclerotic dentin with the bur, the remaining dentin was etched with 37% phosphoric acid and the self-etch adhesive systems Clearfil SE and Xeno III were applied, respectively. Lesions were evaluated at baseline, and restorations after 3 months, 1 year and 2 years using modified USPHS criteria. RESULTS: After 2 years, no significant difference was found between the retention rates of the groups (p >0.05). Although groups CL and SI showed significantly better marginal adaptation than group XE (p<0.05) at 2 years, no significant difference was found between the marginal adaptation of the groups SI-B, CL-B and XE-B (p>0.05). After 2 years no significant difference was observed among the marginal staining results of all groups (p>0.05). CONCLUSION: Although all adhesive systems showed similar retention rates, Clearfil SE and Single Bond showed better marginal adaptation than Xeno III after 2 years of follow-up.

ABSTRACT

Recentemente, os lasers de pulsos ultracurtos têm sido avaliados para o preparo dos tecidos duros dentais devido à sua habilidade de reduzir os efeitos térmicos resultantes da irradiação. O objetivo deste estudo foi avaliar a efetividade de união de 3 sistemas adesivos à dentina preparada com o laser de femtossegundos (10-15 s). Para isso, 60 terceiros molares foram preparados com o laser de femtossegundos (1.450 nm; 0,3W; 100.000Hz) ou ponta diamantada, divididos em 3 subgrupos (n=10) restaurados utilizando o adesivo condicione e lave de 3 passos Optibond FL (OFL; Kerr), o autocondicionante de 2 passos Clearfil SE (CSE; Kuraray) ou o autocondicionante de 1 passo G-aenial Bond (GAB; GC). Após 1 semana de armazenamento em água, os dentes foram seccionados (1,0x1,0 mm) e testados imediatamente e após 6 meses de armazenamento. Vinte e quatro amostras adicionais foram preparadas com o laser ou ponta diamantanda conforme descrito acima. Seis amostras foram seccionadas em espécimes menores e aleatoriamente preparados para observação das características morfológicas da superfície (n=3) e subsuperfície (n=3) em Microscópio Eletrônico de Varredura (MEV) (n=3) e Microscopia Eletrônica de Transmissão (MET) (n=3). As outras 18 amostras foram restauradas com os sistemas adesivos supramencionados, seccionados em espécimes menores e distribuídos para avaliação da interface adesiva em MEV (n=3) e padrão de nanoinfiltração após 1 semana (n=2) e 6 meses (n=2) em MET. O modelo linear de efeitos mistos (=0,05) revelou um efeito significante dos fatores adesivo, armazenagem e a interação preparo x armazenagem na resistência de união. Todos os adesivos apresentaram uma redução significante da resistência de união após 6 meses de armazenamento (p<0,0001) para ambos os tipos de preparo. Com relação ao sistema adesivo, a única diferença verificada foi o maior valor de resistência de união do GAB quando aplicado sobre as superfícies irradiadas (56,71±10,72 MPa) quando comparadas com as preparadas com ponta diamantada (43,14±14,93 MPa) após 1 semana de armazenamento. A análise de fratura revelou fraturas mistas, adesivas e coesivas em dentina para ambos os métodos de preparo com um discreto aumento do número de fraturas mistas após 6 meses de armazenamento exceto para o GAB, que apresentou principalmente fraturas adesivas e mistas. A análise em MEV revelou uma fina camada de debris sobre a superfície da dentina irradiada e a hibridização com todos os sistemas adesivos resultou num íntimo contado da interface adesiva com o substrato. A análise em MET da subsuperfície irradiada revelou fibrilas colágenas preservadas. A análise da interface adesiva revelou um aumento dos depósitos de prata na camada híbrida para todos os adesivos, após 6 meses de armazenamento, independentemente do tipo de preparo. Apenas o GAB apresentou maiores depósitos de prata após 1 semana de armazenamento para as superfícies preparadas com ponta diamantada. A irradiação com o laser de femtossegundos não reduziu a receptividade da dentina aos sistemas adesivos, resultando em valores de resistência de união similares aos obtidos com a ponta diamantada; além de uma ótima interação do substrato com os sistemas adesivos e deposições de prata similares àquelas obtidas com a ponta diamantada.


Recently, the applicability of ultrashort pulsed lasers has been studied for dental hard-tissue irradiation because of its reduced residual thermal damage. The aim of this study was to determine the bonding effectiveness of 3 contemporary adhesive systems to dentin prepared by femtosecond (10-15 sec) laser-irradiation. For this, 60 third molars were prepared either by the femtosecond laser (1450 nm; 0.3W; 100,000Hz) or by bur, randomly divided into 3 subgroups (n=10) and restored with a composite using either the 3-step etch-and-rinse adhesive Optibond FL (OFL; Kerr), the 2-step self-etch adhesive Clearfil SE (CSE; Kuraray) or the 1-step self-etch adhesive G-aenial Bond (GAB; GC). After 1-week water storage, the teeth were sectioned (1.0x1.0 mm) and half of the specimens were tested until failure using a micro-tensile bond-strength (TBS) approach. The other half was stored in water and tested after 6 months. Twenty-four additional dentin samples were exposed to the diamond bur and the laser, following the same abovementioned parameters. Three samples of each treatment were sectioned in smaller specimens in order to be characterized for surface (n=3) and cross-sectional morphological analysis using Scanning Electron Microscopy (SEM) (n=3) and Transmission Electron Microscopy (TEM) (n=3). The other eighteen samples were hybridized with the abovementioned adhesive systems, sectioned in smaller samples and distributed for evaluation of the interface under SEM (n=3) and nanoleakage distribution for 1 week (n=2) and 6 months (n=2) under TEM. Linear mixed effect model (=0.05) revealed a significant effect of the factors adhesive, storage and the interaction of preparation x storage on the TBS.

ABSTRACT

O objetivo deste estudo foi revisar a literatura para evidenciar os aspectos científicos e técnicos envolvidos no selamento de lesões de cárie em superfícies oclusais e proximais. Após a análise dos artigos referenciados, encontrados a partir de busca eletrônica e manual, observou-se que há trabalhos estudando a possibilidade de paralisação da lesão oclusal e proximal com procedimentos não invasivos. Diversos estudos são discutidos e o passo a passo da técnica para selamento proximal é abordado. O selamento parece ser uma ferramenta eficaz na paralisação da lesão cariosa em superfícies proximais, o que vem a ser um ganho de estrutura dental sadia em relação às técnicas invasivas. Apesar deste procedimento se mostrar bastante promissor em superfícies proximais, outros estudos clínicos devem ser realizados.


This study reviewed the literature to evidence the scientific and technical aspects involved in sealing occlusal and proximal carious lesions. Many studies report that it is possible to arrest the progression of occlusal and proximal caries with noninvasive procedures. Thus, sealing may accomplish this effectively and preserve more dental tissue than traditional caries treatment methods. The technique used for sealing proximal caries is discussed step-by-step. This technique seems very promising for arresting caries progression in proximal surfaces but more studies are necessary since studies on the topic are few.

El recubrimiento pulpar directo e indirecto, desde el punto de vista biológico y clínico, representa importantes mecanismos para el mantenimiento de la vitalidad pulpar; sin embargo aún es considerado un tema controversial en la clínica odontológica, debido a la complejidad del diagnóstico y a la delicada conducta terapéutica necesaria para obtener éxito en el tratamiento clínico. En tal sentido, este artículo realizará un abordaje biológico y clínico del Recubrimiento Pulpar Directo e Indirecto con la intención de resaltar la importancia fundamental del diagnóstico clínico y radiográfico de la condición pulpar, enfatizando las indicaciones y contra indicaciones de ambos tratamientos. Adicionalmente serán materia de discusión los materiales comúnmente utilizados para la protección del complejo dentinopulpar y las nuevas perspectivas en esta área


Direct and indirect pulp capping represents important mechanisms, from the point of clinical and biological aspects to maintain the pulp vitality. However, it still a controversy issue at dental field, once the complexity diagnosis and the accurately therapeutics are need to achieve the success of clinical treatment. Seeing that, this article outlines the broad of the biological and clinical approach of the direct and indirect pulp capping. Emphasizing fundamentally the importance of the corrected diagnose of clinical and radiographic pulp condition, further the indications and contra-indications of both treatments. Furthermore, it will be discussed regarding the classical dental materials used to dentin-pulp protection and contemporary perspectives in this area

Background: Dentin hypersensitivity is a recurrent condition causing discomfort and sometimes pain to the patient, which also deters him from maintaining adequate oral hygiene. Home care and office measures are used for treatment of this malady. Aim: To evaluate and compare the efficacy of 2 treatment modalities for dentinal hypersensitivity, iontophoresis with acidulated phosphate gel (APF) gel, and dentin-bonding agent application. Materials and Methods: This split mouth randomized clinical study recruited subjects with a history of hypersensitivity with at least 2 teeth, verified by a light stroke with a dental explorer along the cervical third of the teeth. The patients were subjected to a 1-s air blast and cold water stimuli and their responses were recorded on a verbal rating scale. A total of 30 sites from 15 patients were divided into Group A-1.23% APF gel iontophoresis; and Group B-aqueous solution of hydroxyl-ethyl-methacrylate and glutaraldehyde. The teeth were evaluated immediately after the treatment and at the end of 2 weeks. In case of failure, the tooth was retreated with the same agent as before. Results: The results were statistically analyzed using Kruskal-Wallis analysis of variance and Mann-Whitney U tests. There was no statistically significant difference between the groups at the end of 2 weeks. However, Group A was more effective clinically, with fewer number of failures compared with Group B. Conclusion: Both the agents showed a statistically significant reduction in sensitivity compared with baseline; however, APF gel iontophoresis was more effective in reducing hypersensitivity over a longer time period.
